Summary

Plateau pressure (P(pl)) and resistance to outflow (R(out)) from lumbar infusion tests (LIT) are not superior to P(pl) alone for predicting shunt surgery outcomes in normal pressure hydrocephalus (NPH). R(out) may miss patients who benefit from surgery.

Background:

  • Normal pressure hydrocephalus (NPH) is a neurological condition often treated with shunting.
  • Predicting shunt surgery outcomes is crucial for patient selection.
  • The constant rate lumbar infusion test (LIT) provides several pressure-related parameters.

Purpose of the Study:

  • To evaluate the clinical utility of plateau pressure (P(pl)), resistance to outflow (R(out)), pulse-pressure amplitude (P(plA)), and rate of pressure increase (v(P)) from LIT as predictors of shunt surgery success.
  • To determine if these parameters offer advantages over P(pl) alone in patient selection for NPH surgery.

Main Methods:

  • Preoperative LIT recordings were analyzed for P(pl), P(plA), v(P), and R(out) in 55 patients.
  • Patient functional status (gait, memory, spatial capacity, reaction ability) was assessed before and 6 months after shunt surgery.

  • 78% of patients showed improvement after surgery.
  • No statistically significant differences were found in P(pl), R(out), P(plA), or v(P) between improved and unimproved patients.

Conclusions:

  • Neither v(P) nor P(plA) provide additional predictive value beyond P(pl) for selecting NPH patients for shunt surgery.
  • R(out) calculations from LIT do not offer an advantage over steady-state P(pl) and may increase the risk of excluding suitable candidates.
