Summary

A rare post-hemipelvectomy hernia developed 33 years after surgery, impairing prosthesis use. Surgical repair with a polypropylene mesh successfully restored function and prevented recurrence, highlighting mesh efficacy for this complication.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Reconstructive Surgery

Background:

  • A rare complication following hemipelvectomy, a major surgical procedure for pelvic tumors, is the development of a post-surgical hernia.
  • Such hernias can significantly impair the function of prosthetic devices, impacting patient mobility and quality of life.

Observation:

  • A case is presented of a patient who developed a post-hemipelvectomy hernia 33 years after the initial surgery for femur fibrosarcoma with inguinal metastases.
  • The hernia occurred in the amputation stump, severely limiting the use of his Canadian prosthesis.

Findings:

  • The hernia was successfully repaired using a polypropylene mesh placed in a subaponeurotic position.
  • Postoperative management included drainage of a seroma, with the patient regaining functional use of his prosthesis within two months.

Related Experiment Videos

  • One year post-repair, there was no evidence of hernia recurrence.
  • Implications:

    • This case adds to the limited literature on post-hemipelvectomy hernias, with only a few documented cases and even fewer detailing successful mesh repair.
    • The successful outcome suggests that polypropylene mesh repair is a viable and effective option for managing this rare but debilitating complication.
    • Further research and case reporting are warranted to establish optimal surgical strategies for post-hemipelvectomy hernia repair.
