This study details a novel approach for treating metastatic differentiated thyroid carcinoma. Radioguided surgery successfully located and removed occult metastases missed by initial diagnostic scans.

- Metastatic foci were successfully localized in the central neck compartment using radioguided surgery.
- Pathology confirmed 2 of 12 lymph nodes and a small focus in the fat were positive for metastasis.
- This represents a novel combination of provocative imaging, radioiodine therapy, and radioguided surgery.
Implications:
- This approach may offer a curative option for patients with image-negative metastatic DTC.
- Radioguided surgery can enhance the detection and resection of subtle metastatic disease.
- Further research is warranted to validate this combined strategy in a larger cohort.
