Purpose of the Study:

  • To correlate regional cerebral blood flow (rCBF) changes with language performance in subcortical aphasia patients.
  • To investigate longitudinal changes in brain activity and language recovery over one year.

Main Methods:

  • Positron emission tomography (PET) scans were performed twice, one year apart, on seven aphasic patients during a word-generation task.
  • Statistical Parametric Mapping (SPM2) was used to analyze regional cerebral blood flow (rCBF) changes and correlate them with language performance and time from stroke.

  • Initial correlations between rCBF and stroke delay involved left ventral temporal and mesial frontal cortices.
  • Language performance correlated with left dorsal frontal, temporal, parietal regions, and left ventral temporal cortex.
  • Over one year, language performance improved with increased bilateral perisylvian rCBF; best performers showed increased left ventral temporal and right middle temporal gyrus activity.

Conclusions:

  • Follow-up neuroimaging demonstrated expected language improvement and increased activation in classical language areas and their counterparts.
  • The anterior left inferior temporal gyrus was consistently implicated, suggesting a role in lexical semantic processing and potential disconnection within the temporal cortex.
