Background:

  • A 77-year-old completely edentulous female patient diagnosed with malignant carcinoma underwent resection of the upper left posterior maxilla.
  • Conventional complete dentures were modified into an immediate surgical obturator using resin to address functional deficits post-resection.

Observation:

  • The surgical obturator facilitated immediate functional restoration.
  • Following the healing period, a definitive obturator prosthesis was fabricated to further enhance speech and masticatory capabilities.

Findings:

  • The definitive obturator featured a buccal flange design to minimize weight and utilized flat cusped posterior teeth for reduced stress on the residual ridge and improved occlusal balance.
  • The patient reported satisfaction with the recovery of chewing and pronunciation, noting speech and function similar to her pre-operative condition.

Implications:

  • Maxillary obturator prostheses are effective in restoring speech and chewing function after partial maxillectomy in edentulous patients.
  • This prosthetic rehabilitation approach can significantly improve the quality of life for patients undergoing extensive oral cancer surgery.
