Pruritus in hemodialysis patients

Background:

• Pruritus is a common and distressing symptom for patients undergoing maintenance hemodialysis (HD).
• The exact causes of pruritus in HD patients remain poorly understood, hindering effective treatment development.

Purpose of the Study:

• To determine the prevalence and intensity of pruritus among HD patients in Tehran, Iran.
• To investigate the correlation between pruritus and various clinical and laboratory parameters in this population.

Main Methods:

• A study involving 167 patients undergoing maintenance HD at three outpatient units.
• Assessment of itch severity (mild, moderate, severe) and evaluation of parameters including age, sex, xerosis, neuropathy, dialysis duration, atopy history, and lab values (hematocrit, creatinine, urea, calcium, phosphorus, PTH, alkaline phosphatase).

• Pruritus was reported by 41.9% of patients, with 31.4% experiencing intensified itching during or after dialysis.
• No significant differences were observed in serum levels of creatinine, urea, calcium, phosphorus, alkaline phosphatase, PTH, or hematocrit between pruritic and non-pruritic groups.
• Clinical neuropathy was significantly more prevalent in patients with pruritus (63.8%) compared to those without (42.1%), with a p-value of 0.006.

Conclusions:

• Clinical neuropathy emerged as the sole significant factor associated with pruritus in this cohort of hemodialysis patients.
• These findings highlight the need for further investigation into nerve function and neurotransmitter roles in dialysis-related pruritus.
• The results suggest potential avenues for developing novel therapeutic strategies targeting neuropathy to alleviate itching in HD patients.
