Observation:

  • A 10-year-old boy presented with a lung abscess secondary to Staphylococcus infection originating from a finger panaritium.
  • Diagnosis was confirmed via roentgenographic examination showing a cavitary lesion with fluid level and alveolar infiltration.

Findings:

  • Conservative treatment with Vancomycin and other antifungals/antibiotics over 40 days led to abscess resolution.
  • Serial chest X-rays demonstrated gradual diminution of the abscess, with a residual air cyst noted.
  • Associated osteitis of the second phalange in the affected finger also showed improvement.

Implications:

  • This case underscores the efficacy of prolonged conservative antibiotic therapy in pediatric lung abscess.
  • It emphasizes the importance of identifying and treating the primary source of infection.
  • Surgical intervention is reserved for complex or recurrent cases in pediatric patients.
