Area of Science:

  • Infectious Diseases
  • Pulmonology
  • Mycology

Background:

  • Cryptococcal pneumonia is rare in immunocompetent individuals.
  • Treatment guidelines and duration are undefined for this condition.
  • Cavitating pulmonary cryptococcosis presents unique clinical challenges.

Purpose of the Study:

  • To evaluate the efficacy and safety of oral fluconazole for cryptococcal pneumonia in immunocompetent hosts.
  • To explore the role of serum cryptococcal antigen (SCA) monitoring in treatment management.

Main Methods:

  • Retrospective analysis of three immunocompetent patients with cavitating cryptococcal pneumonia.
  • Treatment with oral fluconazole.
  • Monitoring of clinical, radiological, and serological parameters, including SCA levels.

Related Experiment Videos

Main Results:

  • All three patients showed significant clinical, radiological, and serological improvement.
  • Fluconazole treatment was continued until negative SCA levels were achieved.
  • Manifestations like hemoptysis and cavitating pneumonia indicated potentially severe disease.

Conclusions:

  • Oral fluconazole is an effective and safe treatment option for cryptococcal pneumonia in immunocompetent hosts.
  • Monitoring SCA levels may indicate disease burden and guide treatment duration.
  • Extended treatment duration appears beneficial, outweighing potential risks.
