Summary

Limited evidence exists for using high-volume fluids or diuretics to treat acute ureteric colic. More research is needed to determine the safety and efficacy of these interventions for expediting kidney stone passage.

Background:

  • Acute ureteric colic causes severe pain.
  • High-volume fluids and diuretics are theoretical treatments to expedite kidney stone passage.

Purpose of the Study:

  • To evaluate the benefits and harms of diuretics and high-volume fluid therapy.
  • To assess treatment for adult patients with uncomplicated acute ureteric colic.

Main Methods:

  • Searched multiple databases (Cochrane Renal Group, CENTRAL, MEDLINE, EMBASE) up to July 2004.
  • Included randomized controlled trials (RCTs) and quasi-RCTs.
  • Two reviewers assessed trial quality and extracted data; random effects model used for analysis.

  • One RCT with 60 participants was identified.
  • Compared no fluids versus 3 liters of IV fluids over six hours.
  • No significant differences in pain, surgical stone removal, or cystoscopic manipulation were found.

Conclusions:

  • No credible evidence was found for diuretics or high-volume fluid therapy in acute ureteric colic.
  • Further research is recommended to examine the safety and efficacy of these treatments.
  • Investigating these interventions could impact facilitating kidney stone passage.
