Community-acquired infections from vancomycin-resistant enterococci (VRE) are rare. A case involved VRE infecting a patient through a burn caused by topical herbal leaf application, highlighting potential community reservoirs.

Area of Science:

  • Infectious Diseases
  • Microbiology
  • Antimicrobial Resistance

Background:

  • Vancomycin-resistant enterococci (VRE) are significant pathogens, primarily causing hospital-acquired infections.
  • VRE possess the ability to transfer resistance genes, complicating treatment and increasing public health concerns.

Observation:

  • A rare case of community-acquired infection caused by vancomycin-resistant Enterococcus faecium is presented.
  • The infection originated from a buccal space abscess treated with topical herbal leaves, causing a skin burn.

Findings:

  • A pure culture of vancomycin-resistant Enterococcus faecium (VanA phenotype) was isolated from the abscess pus.
  • The organism exhibited high-level resistance to vancomycin (MIC >256 µg/mL) and teicoplanin.

Related Experiment Videos

Implications:

  • This case suggests herbal leaves as a potential source for VRE contamination and infection in the community.
  • Further research into community VRE prevalence in humans and animals is crucial to identify silent reservoirs and mitigate public health risks.
