This case study highlights a reversible form of stress-induced cardiomyopathy presenting with ST-segment elevation myocardial infarction (STEMI)-like features. The patient experienced dynamic left ventricular outflow tract obstruction upon dobutamine stress testing, resolving with medical management.

Background:

  • Takotsubo cardiomyopathy (TTC) is a form of non-ischemic heart muscle dysfunction.
  • It can mimic acute myocardial infarction, presenting with ST-segment elevation and regional wall motion abnormalities.
  • Dynamic left ventricular outflow tract (LVOT) obstruction is an uncommon but recognized complication.

Observation:

  • A 78-year-old woman presented with chest oppression and ST-segment elevation myocardial infarction (STEMI)-like electrocardiogram findings.
  • Initial ventriculography revealed apical akinesis and basal hyperkinesis with significant pressure gradients, suggesting dynamic LVOT obstruction.
  • These abnormalities resolved spontaneously within 16 days.

Findings:

  • Recatheterization demonstrated normal left ventricular wall motion and no pressure gradients at rest.
