Summary

Patients with ongoing pain therapy using intrathecal morphine or spinal cord stimulation (SCS) need implant tests and imaging for new lumbar pain. Reoperation for recurrent disc herniation significantly relieved preoperative pain in six cases.

Area of Science:

  • Neurosurgery
  • Pain Management
  • Neurology

Background:

  • Palliative pain therapy often involves intrathecal morphine or spinal cord stimulation (SCS).
  • Recurrent disc herniation can cause persistent lumbar pain, complicating management.

Observation:

  • Patients on palliative pain therapy experiencing new or altered lumbar pain require evaluation.
  • Diagnostic imaging and functional implant tests are crucial for assessing these patients.

Findings:

  • Six cases of recurrent disc herniation treated with reoperation are presented.
  • Reoperation led to significant reduction in preoperative pain complaints.

Implications:

  • This suggests reoperation for recurrent disc herniation is an effective strategy for refractory lumbar pain in patients on palliative therapy.
  • Highlights the importance of re-evaluating implant function and considering surgical intervention for recurrent disc herniation.
