Summary

A rare case of orbital myositis, an eye muscle inflammation, occurred in a man with ulcerative colitis. Treatment with steroids improved both his eye and intestinal conditions, suggesting a link between these disorders.

Background:

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
  • Orbital myositis is a rare inflammatory condition affecting eye muscles.
  • A potential association between UC and orbital myositis is infrequently reported.

Observation:

  • A 32-year-old man with a 3-year history of UC presented with 2 months of progressive left eye proptosis.
  • Imaging revealed isolated left superior rectus muscle enlargement without visual impairment or diplopia.
  • Systemic and ocular examinations ruled out other causes.

Findings:

  • The patient responded well to a short course of systemic steroids.
  • Both proptosis and UC symptoms improved rapidly.

  • After 1 year, orbital myositis was stable, and UC was managed with mesalazine.
  • Implications:

    • This case highlights a rare association between orbital myositis and ulcerative colitis.
    • Concomitant management of both conditions may be beneficial.
    • Further research is warranted to understand the underlying pathophysiology linking these disorders.
