Background:

  • Endothelins are potent vasoconstrictor peptides.
  • Endothelin-1 (ET-1) is produced by endothelial cells, while endothelin-3 (ET-3) originates from neuronal tissue.
  • ET-1-mediated vasoconstriction may exacerbate ischemic neuronal damage.

Purpose of the Study:

  • To measure plasma levels of endothelin-1 and endothelin-3.
  • To investigate the role of endothelins in acute nonhemorrhagic cerebral infarction.

Main Methods:

  • Radioimmunoassay was used to measure plasma ET-1 and ET-3 levels.
  • 16 patients with acute nonhemorrhagic cerebral infarction were studied within 72 hours of stroke onset.
  • A control group of 13 age-matched subjects was included for comparison.

Main Results:

  • Plasma ET-1 levels were markedly elevated (fourfold) in patients compared to controls (median 11.7 pg/ml vs. 2.56 pg/ml, p < 0.0001).
  • Higher ET-1 levels were observed within the first 24 hours post-stroke.
  • A trend suggested elevated ET-1 levels correlated with more severe neurological deficits.
  • ET-3 levels were below 0.5 pg/ml in all subjects.

Conclusions:

  • Ischemic stroke is associated with acute, significant increases in plasma ET-1.
  • Elevated ET-1 may result from enhanced production by damaged endothelial cells in infarcted brain tissue.
  • Local ET-1 leakage could cause prolonged vasoconstriction of collateral vessels, potentially harming stroke pathogenesis and outcome.
