Summary

Ghrelin levels are similar in medullary thyroid carcinoma (MTC) patients and healthy individuals, indicating it is not a reliable disease marker. However, ghrelin may be a minor product of C cells, as suggested by increased levels during abnormal calcitonin responses.

Background:

  • Ghrelin, a gastrointestinal hormone, plays a role in metabolic functions and has been found in neuroendocrine tissues, including medullary thyroid carcinomas (MTCs).
  • Elevated calcitonin (CT) levels are indicative of MTC and related conditions.
  • The potential role of ghrelin as a biomarker in thyroid disorders requires investigation.

Purpose of the Study:

  • To evaluate circulating ghrelin levels in patients with MTC and nontoxic goitre (NTG) with elevated CT.
  • To determine if ghrelin can serve as an additional diagnostic marker for MTC.
  • To investigate the relationship between ghrelin levels, CT levels, and MTC status.

Main Methods:

  • Plasma ghrelin and CT levels were measured in 22 MTC patients, 12 NTG patients, and 15 healthy controls.

  • Ghrelin expression was examined in MTC tissue samples using immunohistochemistry.

    • No significant correlation was found between ghrelin levels and CT levels, thyroid disease, or prior thyroidectomy.
    • Pentagastrin administration induced a modest increase in ghrelin levels, more pronounced in patients with abnormal CT responses.

    Conclusions:

    • Ghrelin levels do not appear to be a useful marker for diagnosing MTC or NTG.
    • The observed increase in ghrelin during abnormal CT responses suggests a potential, albeit minor, role for C cells in ghrelin production.
    • Further research is needed to fully elucidate the role of ghrelin in thyroid C-cell function and disease.
