Summary

Primary acquired melanosis with atypia, a conjunctival lesion, carries a high risk of melanoma. Topical mitomycin-C eye drops offer an effective alternative to surgery for treating these pigmented lesions.

Background:

  • Pigmented conjunctival lesions in adults may indicate primary acquired melanosis with atypia.
  • Atypical melanocytes in conjunctival epithelium present a nearly 50% risk of malignant melanoma transformation.

Observation:

  • Conjunctival primary acquired melanosis with atypia is often treated with surgical excision and cryotherapy.
  • Topical chemotherapy using mitomycin-C eye drops is an emerging therapeutic option.

Findings:

  • Topical mitomycin-C eye drops can be a viable alternative to extensive surgery for large or multiple pigmentations.
  • Complete regression of pigmented conjunctival lesions was observed in two patients treated with mitomycin-C eye drops.

Implications:

  • Mitomycin-C eye drops provide a less invasive treatment option for primary acquired melanosis with atypia.
