Extensive persistent pupillary membranes (PPMS) can severely impair vision, but this case shows good vision is possible without surgery. Conservative management, including spectacles and occlusion therapy, effectively treated associated amblyopia and exotropia.

Area of Science:

  • Ophthalmology
  • Pediatric Ophthalmology

Background:

  • Persistent pupillary membranes (PPMs) are congenital iris remnants that can obstruct vision.
  • Extensive PPMs often lead to consideration of aggressive surgical interventions.

Observation:

  • A 6-year-old girl presented with bilateral, dense, pigmented pupillary membranes.
  • The patient also had associated ametropic amblyopia and exotropia.

Findings:

  • Good visual acuity was achieved without any pharmacologic, laser, or surgical treatment for the PPMs.
  • Spectacles and occlusion therapy effectively managed the amblyopia and exotropia.

Implications:

  • Conservative management should be considered prior to surgical intervention for dense PPMs.
