Short bowel syndrome (SBS) can lead to liver failure. Reduced portal venous flow after intestinal resection in piglets correlated with hepatic fibrosis, suggesting a link between circulation changes and liver damage in SBS.

  • Short bowel syndrome (SBS) is a severe condition with progressive liver failure as a major complication.
  • The interplay between hepatic circulation and fibrosis in SBS remains incompletely understood.
  • This study utilized a neonatal piglet model to investigate these relationships.

Purpose of the Study:

  • To examine the impact of massive intestinal resection on hepatic circulation and fibrosis in a neonatal piglet model of SBS.
  • To determine the correlation between changes in portal venous flow and the development of hepatic fibrosis.

Main Methods:

  • Four groups of piglets were used: SBS (80% small bowel resection), functional SBS (bowel bypass), sham (laparotomy), and control (no surgery).
  • Hepatic circulation was measured, and biochemical and morphological analyses of blood and liver tissue were performed.

  • Histological analysis included alpha-smooth muscle actin staining to assess activated hepatic stellate cells.

    • No significant differences in biochemical liver function or overall liver tissue blood flow were observed among groups.
    • Significantly reduced portal venous flow was noted in the SBS group on postoperative days 7 and 14.
    • Mild hepatic fibrosis and increased alpha-smooth muscle actin staining (indicating activated stellate cells) were observed in the SBS group on postoperative day 14.

    Conclusions:

    • Decreased hepatic circulation, particularly portal venous flow, following massive intestinal resection may contribute to progressive liver dysfunction in SBS.
    • Activation of hepatic stellate cells appears to be a key mechanism linking reduced portal flow to liver fibrosis in this SBS model.
