Summary

Hepatitis B virus (HBV) infection rarely causes focal segmental glomerulosclerosis (FSGS). This case highlights successful treatment of HBV-associated FSGS in a child using lamivudine, improving kidney function and viral markers.

Area of Science:

  • Nephrology
  • Hepatology
  • Virology

Background:

  • Hepatitis B virus (HBV) infection is a global health concern, predominantly affecting the liver.
  • While HBV can cause membranous glomerulopathy, its association with focal segmental glomerulosclerosis (FSGS) is infrequently reported.
  • Chronic HBV infection can lead to various kidney diseases, necessitating a comprehensive understanding of its nephropathic potential.

Observation:

  • An 8-year-old boy with chronic HBV infection presented with nephrotic syndrome, initially diagnosed as idiopathic FSGS.
  • The patient received immunosuppressive therapy, which inadvertently increased HBV replication, indicated by elevated liver transaminases.
  • This observation suggested a link between immunosuppression and viral reactivation in the context of HBV-associated kidney disease.

Findings:

Related Experiment Videos

  • The patient's treatment was switched to lamivudine monotherapy, an antiviral agent.
  • Within ten months, significant improvement in nephrotic syndrome was observed.
  • Concurrently, seroconversion of anti-HBeAg and normalization of liver function tests indicated effective viral control.

Implications:

  • This case underscores the rare but significant association between chronic HBV infection and FSGS.
  • Antiviral therapy with lamivudine proved effective in managing HBV-associated FSGS, resolving nephrotic syndrome and controlling viral replication.
  • The findings are particularly relevant for endemic regions of HBV infection, guiding treatment strategies for co-infected patients.
