Background:

  • Optic nerve avulsion is a rare but severe ocular injury.
  • It results from sudden, forceful eye rotation, causing tearing of the optic nerve at the scleral exit.

Observation:

  • Two cases are presented: a 5-year-old boy with bicycle handlebar trauma and a 30-year-old man with a stick injury.
  • Both patients experienced severe vision loss (light perception or no light perception) and pupillary abnormalities.
  • Ophthalmoscopy revealed optic nerve head separation and hemorrhage; ultrasonography and CT confirmed optic nerve thickening and hemorrhage.

Findings:

  • Despite treatment with prednisone, neither patient showed visual improvement.
  • Diagnostic imaging confirmed optic nerve avulsion and associated orbital trauma.
  • Color Doppler ultrasonography indicated reduced blood flow in the central retinal artery.
