Summary
This case study shows that CD4+ cells may control B cell proliferation. HIV infection, which lowers CD4+ cells, might increase lymphoma risk.

Observation:
- A patient with asymptomatic LPL and IgM paraproteinemia acquired HIV infection.
- Following HIV seroconversion, a decrease in CD4+ cell counts and disease progression (increased paraprotein, decreased hemoglobin) were observed.
- Bone marrow analysis revealed reduced CD4+ cells and increased CD138+ cells post-seroconversion.
Findings:
- Highly Active Antiretroviral Therapy (HAART) led to reduced HIV viral load and increased peripheral blood CD4+ counts.
- Treatment also resulted in decreased paraprotein levels and improved hemoglobin.
- The patient's LPL showed signs of progression correlating with CD4+ T-cell depletion.
Implications:
- This case suggests a critical role for CD4+ T-cells in regulating B-cell proliferation.
- CD4+ T-cell suppression in HIV infection may contribute to the higher incidence of lymphoma.
