Background:

  • Idiopathic orthostatic hypotension (IOH) is a chronic condition characterized by a significant drop in blood pressure upon standing.
  • This case highlights a severe presentation of IOH in a 70-year-old woman with a 15-year history of circulatory collapse.

Observation:

  • The patient experienced frequent episodes of circulatory collapse, limiting her ability to remain upright.
  • Autonomic dysfunction was evident, with a pronounced drop in blood pressure (80/50 to 70/30 mmHg) and unchanged pulse rate (60/min) during a Schellong test.
  • Lack of expected rise in serum catecholamines (epinephrine, norepinephrine) upon standing and low urinary vanillylmandelic acid excretion were noted.

Findings:

  • The patient exhibited signs consistent with Shy-Drager syndrome, including gastroparesis, reduced tear and sweat secretion, and urinary incontinence.
  • Inadequate autonomic response to postural changes was confirmed by the Schellong test results and catecholamine levels.

Implications:

  • This case underscores the complex management of severe orthostatic hypotension and associated autonomic failure.
  • Multifaceted symptomatic treatment, including pharmacological and non-pharmacological interventions, improved patient mobility but did not normalize blood pressure responses.
