Summary
We present a rare case of cauda equina syndrome caused by a chronic lumbar epidural hematoma. Surgical treatment led to a full recovery of neurological function.
Area of Science:
- Neurosurgery
- Neurology
- Spinal Cord Medicine
Background:
- Cauda equina syndrome (CES) is a rare neurological condition.
- Chronic epidural hematomas are an uncommon cause of CES.
Observation:
- A 53-year-old male presented with progressive back pain and leg motor deficits.
- No history of trauma, lumbar puncture, or anticoagulant use was reported.
- Magnetic Resonance Imaging (MRI) revealed a chronic epidural hematoma at the L2-L3 lumbar spine level.
Findings:
- Surgical decompression and hematoma evacuation were performed.
- Intraoperative and microscopic examination confirmed the hematoma's nature.
- Histopathology revealed chronic inflammatory changes within the hematoma.
Implications:
- This case highlights chronic epidural hematoma as a treatable cause of CES.
- Early diagnosis and surgical intervention are crucial for favorable outcomes.
- Understanding the etiology of spontaneous epidural hematomas is important for clinical management.
