Frontal fibrosing alopecia, a condition causing eyebrow and hairline loss, was diagnosed in a 75-year-old woman. Standard treatments proved ineffective for her progressive hair loss.

Background:

  • Frontal fibrosing alopecia (FFA) is an increasingly recognized form of scarring alopecia.
  • It is characterized by progressive loss of the frontal hairline and eyebrows.

Observation:

  • A 75-year-old woman experienced a 3-year history of progressive eyebrow hair loss and frontal-parietal hairline recession.
  • Histopathologic examination of biopsy specimens confirmed lichen planopilaris.

Findings:

  • The patient's clinical presentation and biopsy results led to a diagnosis of frontal fibrosing alopecia.
  • Despite treatment with topical glucocorticoids, intralesional triamcinolone, and tacrolimus ointment, her hair loss did not improve.

Implications:

  • This case highlights the potential refractoriness of frontal fibrosing alopecia to conventional therapies.
  • Further research into novel treatment strategies for FFA is warranted.
