Summary

Nerve sparing laparoscopic radical prostatectomy (LRP) using an intrafascial approach shows promising urinary continence and potency rates. Further long-term studies are needed to compare it with interfascial techniques for prostate cancer treatment.

Area of Science:

  • Urology
  • Surgical Oncology

Background:

  • Laparoscopic radical prostatectomy (LRP) is a standard procedure for prostate cancer.
  • Nerve-sparing techniques aim to preserve function post-surgery.
  • The intrafascial approach is a specific nerve-sparing technique for LRP.

Purpose of the Study:

  • To describe the intrafascial nerve-sparing laparoscopic radical prostatectomy (LRP) technique.
  • To evaluate the oncological and functional outcomes, including potency and urinary continence.

Main Methods:

  • Experience with over 2800 LRP cases informed the technique.
  • 425 patients underwent intrafascial LRP between May 2003 and March 2005.
  • Oncological outcomes assessed via pathological examination and PSA levels; functional outcomes via self-questionnaires.

Related Experiment Videos

Main Results:

  • Positive surgical margins observed in 59% of specimens, mostly focal.
  • Continence rate reached 95% at 6 and 12 months.
  • Potency rate was 58.5% at 12 months.

Conclusions:

  • Intrafascial LRP demonstrates satisfactory recovery of urinary continence and sexual function.
  • Long-term oncological outcomes are required to establish its role versus interfascial techniques.
