Hearing

When we hear a sound, our nervous system is detecting sound waves—pressure waves of mechanical energy traveling through a medium. The frequency of the wave is perceived as pitch, while the amplitude is perceived as loudness.

Summary

Bedside screening for hearing impairment in the elderly is effective. Self-reporting hearing loss or failing the whispered-voice test warrants further audiometry for accurate diagnosis.

Area of Science:

  • Gerontology
  • Audiology
  • Diagnostic Medicine

Background:

  • Hearing impairment is common in older adults but often undiagnosed.
  • Early detection is crucial for managing age-related hearing loss.

Purpose of the Study:

  • To evaluate the accuracy and precision of clinical bedside maneuvers for diagnosing hearing impairment in the elderly.

Main Methods:

  • Systematic review of MEDLINE and EMBASE databases (1966-2005).
  • Inclusion of original studies on screening questions and tests for hearing loss.
  • Exclusion of studies with inadequate reference standards or data.

Main Results:

  • Self-reported hearing impairment yielded a likelihood ratio (LR) of 2.5.

  • The Hearing Handicap Inventory for the Elderly-Screening version (HHIE-S) score ≥ 8 had an LR of 3.8.
  • Inability to hear a whispered voice had an LR of 6.1, while normal perception had an LR of 0.03.
  • Conclusions:

    • Self-acknowledgment of hearing impairment necessitates audiometry.
    • The whispered-voice test is recommended for initial screening in those without self-reported impairment.
    • The Weber and Rinne tuning fork tests are not recommended for general screening.
