Background:

  • A 59-year-old immunocompromised male with a history of liver transplant, type 1 diabetes, and poor nutrition presented with acute scrotal pain and swelling.
  • Rapid development of moist, black, foul-smelling lesions on the scrotum and penis indicated a severe necrotizing infection.

Observation:

  • Clinical presentation included rapid progression of necrotic lesions.
  • Diagnostic investigations involved physical examination, blood and tissue cultures, and laboratory tests including full blood count, urea and electrolytes, liver function tests, coagulation profile, and C-reactive protein.
  • Examination under anesthesia was performed to assess the extent of the condition.

Findings:

  • The patient was diagnosed with Fournier's gangrene, a necrotizing fasciitis of the perineum and genitalia.
  • The gangrene originated from an infected cutaneous lesion, exacerbated by the patient's immunocompromised status.

Implications:

  • Management involved immediate resuscitation, triple broad-spectrum antibiotics, and urgent surgical debridement.
  • Multiple debridements and split-skin grafting were necessary.
  • Phallic reconstruction is planned for the future, underscoring the reconstructive challenges in such cases.
