Area of Science:

  • Endocrinology
  • Thoracic Surgery
  • Diagnostic Imaging

Background:

  • Nodular goiter is common in Taiwan, with substernal/intrathoracic variants not infrequent.
  • Intrathoracic goiters are typically secondary, with primary forms being rare.

Observation:

  • A 55-year-old woman presented with a productive cough.
  • Incidental finding of an ectopic goiter in the anterior upper mediastinum.
  • Imaging (X-ray, CT) and 131I-uptake scan confirmed the thoracic lesion.

Findings:

  • Surgical removal via thoracotomy was performed.
  • Pathology confirmed the diagnosis of a primary intrathoracic goiter.
  • The lesion was an ectopic goiter in the anterior upper mediastinum.

Implications:

  • Primary intrathoracic goiter is a rare diagnosis that must be considered in the differential for mediastinal tumors.
  • This case emphasizes the importance of considering ectopic presentations of common thyroid conditions.
  • Highlights the utility of multimodal imaging in diagnosing mediastinal masses.
