Summary

Primary intrathoracic goiters are rare but should be considered in mediastinal tumor diagnoses. This case highlights an incidentally discovered ectopic goiter presenting with a cough.

Nodular goiter is common in Taiwan, with substernal/intrathoracic variants not infrequent.
Intrathoracic goiters are typically secondary, with primary forms being rare.

Observation:

A 55-year-old woman presented with a productive cough.
Incidental finding of an ectopic goiter in the anterior upper mediastinum.
Imaging (X-ray, CT) and 131I-uptake scan confirmed the thoracic lesion.

Findings:

Surgical removal via thoracotomy was performed.
Pathology confirmed the diagnosis of a primary intrathoracic goiter.
The lesion was an ectopic goiter in the anterior upper mediastinum.
