Background:

  • Normal funduscopic examination, fluorescein angiography, and full-field electroretinography (ffERG) can mask central vision deficits.

  • Nine patients presented with unexplained poor central vision and normal ffERG, funduscopy, and angiography.
  • All patients exhibited abnormal MFERG responses localized to the central retina.
  • Most patients reported no family history of similar vision loss, and progression was minimal.

Findings:

  • The observed retinal dysfunction appears distinct from previously described inherited macular dystrophies.
  • Early onset, often in childhood, was reported across the patient cohort.
  • The findings suggest Occult Macular Dystrophy may encompass multiple conditions with varied etiologies.

Implications:

  • MFERG is crucial for diagnosing central vision loss when conventional tests are normal.
  • Further research is needed to elucidate the diverse etiologies of this macular dysfunction.
