Background:
- Peritoneal tuberculosis (TB) can present with symptoms and imaging findings similar to advanced epithelial ovarian carcinoma.
- Disseminated peritoneal TB mimicking advanced-stage endodermal sinus tumor (ESS) has not been previously reported.
Observation:
- An 18-year-old woman presented with pelvic pain and weight loss, exhibiting peritoneal implants and an adnexial mass on imaging.
- Elevated CA125 and alpha-fetoprotein levels initially suggested ESS, but intraoperative findings revealed caseous necrosis consistent with disseminated peritoneal TB.
Findings:
- The patient was diagnosed with disseminated peritoneal tuberculosis, not ESS.
- Widespread hepatic involvement of TB led to hepatic failure and the need for liver transplantation two months post-exploration.
Implications:
- Concomitant peritoneal and hepatic TB can cause false elevations in gynecological tumor markers, potentially leading to misdiagnosis and mismanagement.
- Elevated tumor markers, particularly in premenopausal women, warrant thorough investigation to rule out infectious etiologies like TB.
