Background:

Bacterial endophthalmitis, a severe ocular infection, can lead to vision loss and is often secondary to infections elsewhere in the body.
Infectious endocarditis, a cardiac valve infection, can rarely present with concurrent endophthalmitis.
The co-occurrence of bacterial endophthalmitis and infectious endocarditis is exceptionally rare, particularly in Japan.

Observation:

A 53-year-old woman presented with acute visual impairment, pain, and fever in her left eye.
Initial treatment for endophthalmitis with antibiotics failed, leading to enucleation.
Histopathological analysis revealed group B Streptococcus in the vitreous, and echocardiography showed mitral valve vegetation, confirming infectious endocarditis.

The patient was diagnosed with endophthalmitis secondary to infectious endocarditis caused by group B Streptococcus.
This represents the first reported case of this specific combination in Japan.
Histopathological and echocardiographic evidence confirmed the diagnosis.
