Background:

  • Internal carotid artery atherosclerotic disease can cause ocular symptoms preceding stroke.
  • Common presentations include retinal emboli, amaurosis fugax, and central retinal artery occlusion.

Observation:

  • A 66-year-old man experienced recurrent light-induced amaurosis fugax.
  • This was associated with severe carotid occlusive disease, reduced cerebrovascular reactivity, and prolonged visual evoked potential latency.

Findings:

  • Carotid endarterectomy resolved the patient's symptoms.
  • Post-operative ancillary tests returned to normal.

Implications:

  • Light-induced amaurosis fugax likely has a hemodynamic origin.
  • Recognizing this symptom is vital for identifying patients at high risk of stroke due to carotid stenosis.
