Large hepatic hemangiomas often have irregular edges and adjacent hemangioma-like vessels (HLVs). These HLVs appear to be part of the cavernous hemangioma (CH) spectrum, challenging traditional views.

Background:

  • Cavernous hemangiomas (CH) are typically characterized as solitary, well-circumscribed liver lesions with a distinct fibrous interface.
  • This traditional view may not encompass the full spectrum of histological changes, particularly in larger lesions.

Purpose of the Study:

  • To describe underrecognized histological changes in large hepatic hemangiomas.
  • To investigate the relationship between the main lesion and adjacent vascular structures.

Main Methods:

  • Review of 19 hepatic resections for cavernous hemangiomas.
  • Application of histochemical and immunohistochemical stains (ER/PR, MIB-1, alpha-smooth muscle actin, collagen IV, elastic Van Gieson).

  • Large CHs (mean 16.6 cm) frequently exhibited irregular interfaces (84%) rather than well-defined fibrous capsules.
  • Hemangioma-like vessels (HLVs) extending beyond the main CH were observed in 79% of cases.
  • Histochemical and immunohistochemical profiles of CH and HLVs were similar, showing predominantly collagenous walls, minimal smooth muscle/elastic fibers, and low proliferation rates (<5/100 endothelial cells).

Conclusions:

  • Irregular borders of CH and the presence of adjacent HLVs are underrecognized features.
  • HLVs share similar histological characteristics and proliferative rates with CH, suggesting they are part of the CH spectrum.
  • These findings challenge the long-standing description of CH as uniformly well-circumscribed lesions.
