Summary

Two-thirds of patients with mild chronic hepatitis C experience liver fibrosis progression within 5-10 years. Advanced fibrosis or cirrhosis developed in one-third of those with initial stage F1, suggesting antiviral therapy is beneficial.

Background:

  • The natural history of chronic hepatitis C (CHC) with minimal liver fibrosis is not well understood.
  • There is ongoing debate regarding the risk of fibrosis progression and the necessity of antiviral treatment in these patients.

Purpose of the Study:

  • To investigate the rates and predictors of liver fibrosis progression in patients with initially mild CHC.
  • To determine the long-term outcomes of untreated CHC patients with no or minimal fibrosis.

Main Methods:

  • Prospective follow-up of 106 untreated CHC patients with initial fibrosis stage F0/F1 (METAVIR score).
  • Repeated liver biopsies were performed after a mean interval of 7.8 years.
  • Comparison of fibrosis progression rates between patient groups based on baseline and follow-up parameters.

  • Sixty-four percent (60.4%) of patients showed fibrosis progression.
  • Progression to advanced fibrosis (F3) or cirrhosis occurred in 36% of patients initially staged as F1.
  • Fibrosis progression was significantly associated with older age, elevated alanine aminotransferase (ALT) levels, histological activity, steatosis, and alcohol consumption.

Conclusions:

  • Liver fibrosis progresses in a majority of patients with initially mild CHC over a 5-10 year period.
  • Advanced fibrosis and cirrhosis are significant risks for patients with initial F1 stage.
  • Antiviral therapy should be considered for patients with mild CHC due to the observed progression rates.
