  • Thick, unstable burn scars on the face and scalp lead to functional and esthetic deficits.

Observation:

  • A 37-year-old male with extensive facial and scalp burn scars required large skin grafts (30x35 cm).
  • Available donor sites were limited to the back and buttocks.

Findings:

  • A bilateral parascapular free flap was pre-expanded to maximize tissue availability.
  • This technique successfully resurfaced the entire face and anterior scalp with healthy skin and soft tissue.

Implications:

  • Pre-expansion of free flaps offers a viable solution for large-scale facial and scalp reconstruction.
