Endoscopic thyroid lobectomy via a lateral approach (ETLA) is a safe and effective minimally invasive technique. This study shows ETLA provides excellent visualization and outcomes for selected thyroid nodules.

Area of Science:

  • Minimally Invasive Surgery
  • Endocrine Surgery
  • Surgical Oncology

Background:

  • Endoscopic thyroid surgery is a feasible alternative to open procedures.
  • The lateral approach for endoscopic parathyroidectomy has been successfully utilized.
  • This experience extends the lateral approach to endoscopic thyroid lobectomy.

Purpose of the Study:

  • To present the experience and outcomes of endoscopic thyroid lobectomy via a lateral approach (ETLA).
  • To evaluate the safety and efficacy of ETLA for selected thyroid nodules.
  • To assess the feasibility of ETLA in a clinical setting.

Main Methods:

  • A retrospective review of patients undergoing ETLA between January and December 2004.
  • Inclusion criteria: solitary nodules (<3 cm) with suspicious fine-needle biopsy or toxic nodules.

Related Experiment Videos

  • Exclusion criteria: prior neck surgery or radiation exposure; postoperative vocal cord and calcium checks performed.
  • Main Results:

    • 38 patients underwent ETLA, representing 5.1% of all thyroid procedures.
    • Indications included suspicious FNB (36 patients) and toxic nodules (2 patients).
    • Mean operative time was 102 minutes; all recurrent laryngeal nerves were identified; no permanent complications occurred.

    Conclusions:

    • Endoscopic thyroid lobectomy via a lateral approach (ETLA) provides excellent visualization of vital structures.
    • ETLA is a safe alternative to conventional thyroid lobectomy for selected patients.
    • The lateral approach is a viable option for minimally invasive thyroid surgery.
