Summary
Trans-pancreatic sphincterotomy (TS) provides effective biliary tract access when routine methods fail. This technique offers a high success rate with a complication risk comparable to other pre-cut methods.
Area of Science:
- Gastroenterology
- Endoscopic Retrograde Cholangiopancreatography (ERCP)
Background:
- Inaccessible bile ducts pose challenges in ERCP procedures.
- Trans-pancreatic sphincterotomy (TS) is an underutilized technique for gaining biliary access.
Purpose of the Study:
- To evaluate the efficacy and safety of TS in patients with difficult biliary cannulation.
- To present a prospective analysis of TS outcomes.
Main Methods:
- Twenty patients underwent TS between November 2004 and October 2005 due to failed routine cannulation.
- CT-25 sphincterotomes were used to perform cuts towards the bile duct from the pancreatic duct or common bile duct.
Main Results:
- TS was performed in 20 cases (11.3% of sphincterotomies).
- Successful cannulation was achieved in 90% of patients, with 85% immediate success.
- Complication rate was 15%, with no severe complications, pancreatitis, or mortality.
Conclusions:
- TS is an effective method for achieving prompt biliary tract access when standard techniques are unsuccessful.
- The complication rate associated with TS is not higher than that of alternative pre-cut techniques.
