[A system-based approach to drug use in hospital]
Abstract:
Medication errors are the most frequent source of adverse drug effects in the hospital setting. Improvements can be made in four ways:--understanding how systems function within complex technological systems such as hospitals, with a focus on medications; increasing patient involvement, not only in the choice of treatment but also in its implementation;--greater computerization, including all stages of the drug supply chain (from prescription to administration), integrated within the overall hospital computer network, and--encouraging all healthcare professionals to be aware that they are dually accountable: with regard to their personal work, and also to the efficiency and safety of the overall system.
