- Multisite stimulation, involving a second right ventricular electrode, is an emerging strategy for CRT non-responders or those with recurrent symptoms.
Observation:
- A 76-year-old woman with dilated cardiomyopathy experienced symptom recurrence and functional class deterioration (NYHA Class IV) two years after initial CRT.
- The patient was subsequently treated with right ventricular multisite stimulation.
Findings:
- The patient showed a significant reduction in congestive heart failure symptoms following the multisite stimulation procedure.
- Symptom improvement was sustained during the two-month follow-up period post-procedure.
Implications:
- Multisite stimulation may be a viable therapeutic option for select heart failure patients with CRT failure or recurrence.
- This case highlights the potential of advanced CRT techniques to manage refractory heart failure symptoms and improve functional class.
