Functional endoscopic sinus surgery (FESS) is a safe procedure for chronic rhinosinusitis. However, current evidence does not show FESS offers benefits beyond medical treatment alone.

Area of Science:

  • Otolaryngology
  • Surgical Innovation
  • Evidence-Based Medicine

Background:

  • Chronic rhinosinusitis affects all ages and genders.
  • Functional endoscopic sinus surgery (FESS) is a common treatment for medically refractory cases.

Purpose of the Study:

  • To evaluate the effectiveness of FESS for chronic rhinosinusitis.

Main Methods:

  • Systematic review of randomized controlled trials (RCTs).
  • Searched Cochrane, MEDLINE, EMBASE databases.
  • Inclusion criteria: chronic rhinosinusitis diagnosis, symptoms >12 weeks, endoscopic/radiological evidence; Exclusions: immune suppression, cystic fibrosis, Wegener's, prior surgery, malignancy.

Main Results:

  • Three RCTs included.

Related Experiment Videos

  • FESS did not demonstrate superiority over medical treatment +/- sinus irrigation.
  • FESS-based middle meatal antrostomy not superior to traditional inferior meatal antrostomy.
  • Conclusions:

    • FESS is a safe surgical option.
    • Limited evidence suggests FESS provides no additional benefit over medical treatment for chronic rhinosinusitis.
    • Further RCTs with long-term follow-up are needed.
