Summary

A large cardiac myxoma caused syncope and significant coronary artery stenosis. Surgical resection of the mass and bypass surgery successfully treated the patient.

Observation:

  • A 65-year-old male presented with syncope, normal initial investigations, but echocardiography revealed a right atrial mass and tricuspid regurgitation.
  • Coronary angiography showed severe right coronary artery stenosis.

Findings:

  • A large, friable myxoma attached to the atrial septum was resected.
  • Tricuspid valve annuloplasty and coronary artery bypass grafting were performed concurrently.

Implications:

  • Atrial myxomas can cause significant cardiac compromise and coronary artery disease.
  • Multidisciplinary surgical intervention is effective for complex cardiac masses and associated coronary pathology.
