New and emerging treatments for anxiety disorders.

BMJ (Clinical research ed.)·2026

Area of Science:

  • Primary Care Medicine
  • Psychiatry
  • Clinical Psychology

Background:

  • Anxiety disorders are common in primary care (PC) settings, leading to significant patient distress and functional impairment.
  • Detection rates for anxiety disorders in PC are notably low.
  • Current treatment of anxiety disorders in PC often falls short of evidence-based guidelines, with a preference for pharmacologic over psychotherapeutic interventions.

Purpose of the Study:

  • To raise awareness of anxiety disorder prevalence and impairment in PC.
  • To highlight challenges in anxiety disorder detection within PC.
  • To present cognitive-behavioral therapy (CBT) as a viable and effective treatment for anxiety in PC settings.

Main Methods:

  • Review of existing literature on anxiety disorders in primary care.
  • Analysis of treatment biases and guideline adherence.
  • Examination of the components and applicability of CBT in PC.

Main Results:

  • Anxiety disorders significantly impact patients in PC, yet remain underdiagnosed and undertreated.
  • A bias exists favoring medication over psychotherapy, despite CBT's proven efficacy and cost-effectiveness.
  • CBT demonstrates potential for improving patient functioning when integrated into PC.

Conclusions:

  • Primary care settings require enhanced strategies for identifying and managing anxiety disorders.
  • Cognitive-behavioral therapy (CBT) is a well-supported intervention that can be effectively implemented in primary care.
  • Integrating CBT into PC can improve patient outcomes and address the current treatment gap for anxiety disorders.
