Bioelectrical impedance analysis (BIA) reveals body composition changes in kidney transplant recipients with declining kidney function. Patients with worse kidney function showed higher extracellular water and lower intracellular water, similar to chronic kidney disease patients.
Area of Science:
- Nephrology
- Transplantation
- Body Composition Analysis
Background:
- Kidney transplantation (Tx) restores renal filtration but may not fully replicate native kidney function.
- Graft function can decline over time, potentially leading to chronic renal failure.
- Bioelectrical impedance analysis (BIA) is a non-invasive method to assess body composition.
Purpose of the Study:
- To investigate if BIA can detect differences in body compartments among kidney transplant recipients with varying filtration rates.
- To compare body composition in kidney transplant recipients with healthy controls.
Main Methods:
- 38 kidney transplant patients were studied at a median of 75.9 months post-surgery.
- Patients were stratified into three groups based on creatinine clearance (CrCl): good (>65.0), borderline (35.0-60.0), and bad (<35.0 ml/min/1.73 m2).
- BIA parameters including total body water, extracellular water (ECW), intracellular water (ICW), Na:K exchange rate (Nae:Ke), and phase angle were measured. Healthy and hemodialysis patient groups were included for comparison.
Main Results:
- No significant BIA differences were observed between healthy controls and kidney transplant recipients with good CrCl.
- Borderline and bad CrCl groups exhibited significantly higher ECW and lower ICW compared to good CrCl and healthy controls.
- Only the good CrCl group showed differences compared to predialysis values.
Conclusions:
- A well-functioning kidney graft can maintain normal body composition despite potential medication effects (steroids, cyclosporine).
- Mild renal dysfunction post-transplant is associated with body compartment changes, shifting towards a composition seen in chronic renal failure.
- BIA is a valuable tool for monitoring body composition and detecting early signs of renal dysfunction in kidney transplant recipients.
