Summary

Nodular posterior scleritis can mimic choroidal melanoma, presenting as ocular masses. Early differentiation is crucial for appropriate treatment of this inflammatory condition.

Background:

Choroidal melanoma is a significant intraocular malignancy.
Posterior scleritis is an inflammatory condition affecting the posterior part of the eye.

Observation:

A 60-year-old woman presented with blurred vision, proptosis, and an episcleral nodular mass.
Ocular examination revealed a large, nonpigmented choroidal mass.
Imaging studies (ultrasonography, MRI) suggested choroidal melanoma.

Findings:

Biopsy of the episcleral lesion confirmed chronic inflammation consistent with posterior scleritis.
The patient responded well to corticosteroid therapy.
A similar choroidal mass developed in the fellow eye 41 months later.
