Background:

Patients with permanent pacemakers undergoing resuscitation present unique challenges.
Understanding pacemaker behavior during cardiac arrest is crucial for effective treatment.
Previous research on pacemaker interactions with resuscitation is limited.

Observation:

Four cases of witnessed out-of-hospital cardiac arrest in patients with permanent pacemakers were analyzed.
Pacemaker sensing malfunctions occurred in patients not requiring defibrillation.
Defibrillation led to transient pacing, capture, and sensing malfunctions, even with protective measures.

Findings:

Transient pacemaker malfunctions (sensing, pacing, capture) were observed during resuscitation.
Malfunctions were more pronounced with defibrillation, likely due to electrical shock and myocardial dysfunction.
Despite potential life-threatening nature, observed malfunctions did not adversely affect patient outcomes in these cases.

Implications:

Pacemaker behavior during resuscitation requires careful consideration in clinical practice.
Transient pacemaker malfunctions, while concerning, may not alter patient prognosis.
Pacemaker activity can potentially aid in diagnosing underlying cardiac rhythms during arrest.
