Summary

This study identified distinct groups of children with attention-deficit/hyperactivity (ADH) problems. Findings suggest that separating hyperactive-impulsive types of ADHD may not be clinically useful.

Area of Science:

  • Child and Adolescent Psychiatry
  • Developmental Psychology
  • Behavioral Science

Background:

  • Attention-deficit/hyperactivity (ADH) problems are common in referred youth.
  • Understanding homogeneous subgroups within ADH problems is crucial for targeted interventions.
  • Previous typologies of ADHD may not fully capture the heterogeneity of symptoms.

Purpose of the Study:

  • To identify homogeneous groups based on teacher-reported ADH problems in a large referred sample.
  • To examine the co-morbidity patterns across identified ADH problem groups.
  • To evaluate the clinical utility of distinct ADHD subtypes.

Main Methods:

  • Latent class analysis (LCA) was performed on Teacher's Report Form (TRF) data for ADH problems.
  • A sample of 4,422 children and adolescents (ages 6-18) was analyzed.

Related Experiment Videos

  • Co-morbidity with oppositional defiant (OD), conduct, affective, and anxiety problems was assessed.
  • Main Results:

    • Four distinct latent classes emerged: three with combined inattention and hyperactivity-impulsivity, and one with high inattention/low hyperactivity-impulsivity.
    • A distinct group characterized solely by hyperactivity-impulsivity was not identified.
    • Higher levels of ADH problems correlated significantly with increased oppositional defiant, conduct, affective, and anxiety problems.

    Conclusions:

    • The findings challenge the traditional distinction of a hyperactive-impulsive ADHD subtype.
    • A more nuanced understanding of ADH problem heterogeneity is suggested, with combined presentations being common.
    • Comorbidity is significantly associated with the severity of ADH problems, highlighting the need for comprehensive assessment.
