Diffuse large B-cell lymphoma (DLBCL) with spindle cell features is rare and can be misdiagnosed. This case highlights the importance of considering lymphoma in the differential diagnosis of spindle cell tumors, especially in liver masses.

Background:

  • Diffuse large B-cell lymphoma (DLBCL) is a common non-Hodgkin lymphoma.
  • DLBCL with spindle cell morphology is exceptionally rare and poses diagnostic challenges.

Observation:

  • A 70-year-old male presented with a liver mass incidentally detected on CT scan.
  • Initial needle biopsies were diagnosed as suspicious for sarcomatoid hepatocellular carcinoma.
  • Histopathology revealed a mixed epithelioid and spindle cell morphology.

Findings:

  • Immunohistochemistry confirmed tumor cells expressed B-cell markers (CD20, CD79a) and a lymphocytic marker (CD45RB).
  • Spindle-shaped tumor cells were positive for CD20 and negative for alpha-smooth muscle actin.
  • The final diagnosis was primary DLBCL with spindle cell components.
