Summary

Background:

  • Overt hypothyroidism is associated with decreased thyroid echogenicity on ultrasound (US).
  • The relationship between thyroid echogenicity and function in the general population without overt thyroid disease is less understood.

Purpose of the Study:

  • To investigate the association between thyroid echogenicity/irregular echo patterns and thyroid function in the general adult population.
  • To determine if thyroid US can serve as an early indicator of thyroid dysfunction.

Main Methods:

  • A cross-sectional study of 4649 randomly selected adult subjects.
  • Serum thyroid-stimulating hormone (TSH), thyroid hormones, and autoantibodies were analyzed.
  • Thyroid ultrasonography (US) was performed on all participants.

Main Results:

  • Decreased thyroid echogenicity was associated with higher mean TSH levels (1.65 mU/l vs. 1.21 mU/l, P<0.0001).
  • Markedly decreased echogenicity showed a stronger association with elevated TSH (4.20 mU/l, P<0.0001).
  • Decreased echogenicity and irregular echo patterns were more prevalent in subjects with high TSH levels and were associated with a higher risk of thyroid autoantibodies.

Conclusions:

  • Hypoechogenicity on thyroid US is associated with higher serum TSH levels, even in individuals without overt thyroid disease.
  • Decreased echogenicity may be an early sign of thyroid dysfunction.
  • Thyroid US, including assessment of echo patterns, may aid in detecting early and subclinical thyroid dysfunction.
