Summary

Patients with ipsilesional neglect (IN) show a contralesional bias, potentially due to an attentional grasp. Left-sided cues, not right-sided ones, significantly altered line bisection, supporting this hypothesis.

Background:

  • Ipsilesional neglect (IN) is characterized by line bisection towards contralesional space.
  • Existing theories suggest IN may arise from compensatory crossover effects or release of attentional grasp behaviors.

Observation:

  • A patient with IN due to a right medial frontal lesion bisected lines under various cueing conditions (none, left, right, bilateral).
  • The crossover effect hypothesis predicted right-sided cues would influence bisection more than left-sided cues.

Findings:

  • Only left-sided cues induced a significant leftward deviation in line bisection.
  • This finding supports the attentional-approach (grasp) hypothesis over the crossover effect theory.
  • The patient also exhibited ipsilesional extinction to simultaneous stimuli, further supporting the grasp hypothesis.

Implications:

  • The results suggest that ipsilesional neglect may be driven by contralesional attentional grasp mechanisms.
  • Understanding the underlying mechanisms of IN can inform rehabilitation strategies for patients with brain lesions.
  • This study highlights the complexity of spatial attention and neglect phenomena.
