Background:

  • Merkel cell carcinoma (MCC) is a rare, aggressive neuroendocrine skin cancer with high rates of recurrence and metastasis.
  • Early diagnosis and treatment are crucial for managing MCC, but its rarity presents diagnostic challenges.

Observation:

  • A single case of MCC was diagnosed in a 79-year-old woman with a tumor on her upper lip.
  • Following successful cryosurgery and a 7-year disease-free interval, a new tumor appeared in her palatine tonsil.

Findings:

  • The palatine tonsil tumor was histologically characterized as an anaplastic carcinoma with neuroendocrine features.
  • Clinical, histological, immunohistochemical, and genetic analyses indicated the tonsil tumor was a second field cancer, not a metastasis.

Implications:

  • This case highlights the importance of considering secondary field cancers in patients with a history of MCC.
  • Long-term surveillance and comprehensive diagnostic evaluation are essential for distinguishing between recurrence, metastasis, and new primary tumors in MCC survivors.
